Cost and health impacts of adherence to the National Institute for Health and Care Excellence schizophrenia guideline recommendations

Background
Discrepancies between the National Institute for Health and Care Excellence (NICE) schizophrenia guideline recommendations and current clinical practice in the UK have been reported.
AimsWe aim to assess whether it is cost-effective to improve adherence to the NICE schizophrenia guideline recommendations, compared with current practice.
MethodA previously developed whole-disease model for schizophrenia, using the discrete event simulation method, was adapted to assess the cost and health impacts of adherence to the NICE recommendations. Three scenarios to improve adherence to the clinical guidelines were modelled: universal provision of cognitive–behavioural therapy for patients at clinical high risk of psychosis, universal provision of family intervention for patients with first-episode psychosis and prompt provision of clozapine for patients with treatment-resistant schizophrenia. The primary outcomes were lifetime costs and quality-adjusted life-years gained.
ResultsThe results suggest full adherence to the guideline recommendations would decrease cost and improve quality-adjusted life-years. Based on the NICE willingness-to-pay threshold of £20 000–£30 000 per quality-adjusted life-year gained, prompt provision of clozapine for patients with treatment-resistant schizophrenia results in the greatest net monetary benefit, followed by universal provision of cognitive–behavioural therapy for patients at clinical high risk of psychosis, and universal provision of family intervention for patients with first-episode psychosis.
ConclusionsOur results suggest that adherence to guideline recommendations would decrease cost and improve quality-adjusted life-years. Greater investment is needed to improve guideline adherence and therefore improve patient quality of life and realise potential cost savings.

OBJECTIVES/SPECIFIC AIMS: To examine maternal morbidity and its related social determinants among women experiencing homelessness during pregnancy. METHODS/STUDY POPULATION: This study will use an exploratory sequential mixed method design to explore and examine the structural, interpersonal and individual factors contributing to maternal morbidity among a convenience sample of 150 English speaking women experiencing homelessness during a pregnancy within the last 3 years in Baltimore. In the qualitative phase of the study, we will conduct semi-structured interviews with 15 women purposively sampled to refine the relationships between resilience, social determinants of health and multilevel factors that impact maternal morbidities. Factors of interest include prenatal care received, barriers and facilitators to receiving prenatal care, maternal morbidities, social support, and strategies used to manage their condition during this time. Using the findings from the qualitative phase, a quantitative survey will be developed to gather data on topics that emerged in the interviews. In addition, the Housing Instability Index will be used to measure the degree of homelessness as defined by the degree of housing instability in a 6-month period. Using the 25-item Connor-Davidson Resilience Scale, resilience levels among women in the sample will be assessed as a moderating factor in the examination of the relationship between a pregnant woman’s homeless status and maternal morbidity. Descriptive statistics and logistical regression tests will be used to analyze these relationships while controlling for other structural, interpersonal, and individual factors that may be associated with maternal morbidity. RESULTS/ANTICIPATED RESULTS: Qualitatively we expect to gain insight into the relationship between the extrinsic and intrinsic factors impacting maternal morbidities and the health behaviors and practices used by women to manage their pregnancy while homeless. These findings will inform the quantitative survey development and help generalize the quantitative findings. We expect to identify the common morbidities in this population we anticipate that there will be differences in maternal morbidity among the different types of homelessness. Maternal morbidity will be higher among women with a greater degree of homelessness. Resilience will have a moderating effect on the relationship between homelessness and maternal morbidity. DISCUSSION/SIGNIFICANCE OF IMPACT: This study, to our knowledge, is the first to look at maternal morbidity in this population. Additionally, this study seeks to move current research from examining infant outcomes at birth among mothers experiencing homelessness to understanding the maternal morbidities during this period. Long term, good maternal health has significant implications for the health of a mother’s future pregnancies and a risk reduction of adverse chronic conditions. Study results will provide the preliminary knowledge needed to guide further research leading to clinical approaches that promote better maternal health in this population. Lastly, the study findings will inform policy by characterizing the quality and strength of evidence of the adverse maternal health effects associated with the experience of homelessness.

OBJECTIVES/SPECIFIC AIMS: The Life’s Simple 7 (LS7) metric was created by the American Heart Association with the goal of educating the public on seven modifiable factors that contribute to heart health. While it is well documented that these ideal health behaviors lower risk of cardiovascular disease (CVD) in the general population, the association between the LS7 ideal health metrics and end stage renal disease (ESRD) risk has not been examined in a lower socioeconomic population at high risk for both ESRD and CVD. Our objective is to examine the association between the LS7 score and incident ESRD in a cohort of white and black men and women in the southeastern US, where rates of CVD and ESRD are high. METHODS/STUDY POPULATION: The Southern Community Cohort Study recruited ~86,000 low-income blacks and whites in the southeastern US (2002-2009). Utilizing a nested case-control design, our analysis included 1628 incident cases of ESRD identified via linkage of the cohort with the United States Renal Data System (USRDS) from January 1, 2002 to March 31, 2015. Controls (n = 4884) were individually matched 3:1 with ESRD cases based on age, sex, and race. Demographic, medical, and lifestyle information were obtained via baseline questionnaire. The AHA definitions for ideal health were used for non-smoking (never or quit >12 months), body mass index (BMI<25kg/m2) and physical activity (>75 min/week of vigorous physical activity or >150min/week of moderate/vigorous activity). Modified definitions were used for consuming a healthy diet [Healthy Eating Index (HEI10) score>70] and for blood pressure, fasting plasma glucose, and total cholesterol, based on self-reported no history of diagnosis of hypertension, diabetes, and hypercholesterolemia, respectively. The number of ideal health parameters were summed to generate the LS7 score, which ranged from 0-7 with higher scores indicating more ideal health. Adjusted odds ratios (95% confidence intervals) for incident ESRD associated with LS7 score were calculated using conditional logistic regression models, adjusting for income and education. The SCCS ESRD case-cohort dataset will be available by TS 2019 and analyses will be completed to adjust for baseline estimated glomerular filtration rate (eGFR) as a marker of kidney function and to examine whether eGFR modifies the relationship between LS7 and incident ESRD. RESULTS/ANTICIPATED RESULTS: At baseline, mean age was 54 years, 55% (3600) of participants were women, and 87% (5656) were black. A total of 58% (943) of ESRD cases were non-smokers compared to 54% (2633) of controls. ESRD cases had higher prevalence of BMI>25 kg/m2 (81% vs. 74%), hypertension (84% vs. 59%), hypercholesterolemia (48% vs. 34%), and diabetes (66% vs. 22%) compared to controls. A total of 18% (839) of controls and 12% (194) of ESRD cases met ideal exercise recommendations, and 20% of either cases (302) or controls (916) had a HEI10 score above 70. The median LS7 score for controls and ESRD cases was 3 and 2, respectively, and 17% (983) of participants had a low score (0-1) while 2% (105) met 6 or 7 ideal health metrics. Higher LS7 score was associated with lower odds of ESRD (P-trend<0.001). Participants with LS7 score >3 (above median) had 75% reduced odds of ESRD (OR 0.25; 95% CI 0.22, 0.29) compared to those with a score of 2 or less. DISCUSSION/SIGNIFICANCE OF IMPACT: In the SCCS population, the presence of any 3 or more ideal health behaviors is associated with reduced odds of developing ESRD. The components of the LS7 represent important modifiable risk factors that may be targets for future interventions driven by the patient. The attributable risk due to each factor is needed to dissect which ideal behaviors are the most beneficial.

We present a catalogue of 668 major metre-wavelength radio events. The data was collected from dynamic spectra (taken at Dapto, N.S.W. until 1966 and at Culgoora, N.S.W. thereafter) by means of a radio spectrograph, which records the intensity of radio emission as a function of both frequency and time. A description of the major types of metre-wavelength phenomena can be found in Wild et al. (1963), Kundu (1965), Wild and Smerd (1972) and Kruger (1979), and details of the radio spectrograph in Sheridan (1963, 1967) and Labrum (1972).

To date the Culgoora radioheliograph has recorded seven continuum bursts simultaneously at its three operating frequencies of 160, 80 and 43 MHz. In every case the radio emission is found to originate from stationary sources whose heights increase with decreasing frequency. The implications of these new observations are discussed below.

We present an overview of the survey for radio emission from active stars that has been in progress for the last six years using the observatories at Fleurs, Molonglo, Parkes and Tidbinbilla. The role of complementary optical observations at the Anglo-Australian Observatory, Mount Burnett, Mount Stromlo and Siding Spring Observatories and Mount Tamborine are also outlined. We describe the different types of star that have been included in our survey and discuss some of the problems in making the radio observations.

During routine monitoring of active-chromosphere stars in August 1987 an intense radio flare on the FK Comae star HD 32918 was detected by the Parkes radiotelescope. Other observatories were notified, enabling data to be collected at 843 MHz, 1.4 GHz, 8.4 GHz and optical frequencies.
The flare at radio frequencies was largely similar to a previous event on this star, reaching a peak radio power of 5 < 1012 W Hz−1 at 8.4 GHz. During the flare the spectrum varied as ∼ v1.4 and later flattened as the flare progressed. Compared to the previous flare, circular polarization was higher.
Ca II and Hα optical spectra taken at the AAO 3.9-metre telescope show the star to have a highly active chromosphere and a strong wind, with a terminal velocity of ≈230 km s−1. While much of this activity appears to be long lived, there is good evidence for an increase in the strength of the chromospheric lines during the radio flare event.

Introduction
Climate change impacts, and potential adaptive and mitigative responses, have been the subject of major assessments by the Intergovernmental Panel on Climate Change (IPCC), including the Fourth Assessment Report, which was published in 2007. Throughout the assessment process, increasing attention was focused on linkages between climate change responses and sustainable development, in part because climate change adds to the list of stressors that challenge the ability to achieve the ecologic, economic and social objectives that define sustainable development. Development choices, furthermore, can inadvertently result in altered vulnerabilities to climate variability and change, changed patterns of energy and material consumption and, consequently, emissions of carbon dioxide and other pollutants. Risks to human security could increase because global climate change interacts with specific regional stresses, including ecosystem degradation, economic difficulties, exposure to climate-related impacts, low response capacities and weak governance systems at sub-national and national scales (Barnett and Adger, 2007).
Klein et al. (2005) suggest that climate policy can evolve to facilitate the successful embedding of climate change within broader development goals to help reduce vulnerability and insecurity, but the integration of adaptation and mitigation at different operational scales remains a challenge (Jones et al., 2007). Although these responses are widely regarded as complements rather than substitutes, gaps in our understanding of the various capacities that are required to carry out these responses have prevented a truly integrated assessment of response options.

EXAMINATION OF THE BRAIN AT AUTOPSY – AN APPROACH
This chapter considers the pathological changes that may be found in brains examined in non-forensic autopsies, where the pathology falls within the general heading of ‘natural causes’. Following recent organ-retention issues hospital consented autopsy practice has undergone marked change, resulting in a decline in the proportion of hospital deaths that come to autopsy. A recent audit in this hospital showed a fall in the autopsy rate for in-hospital deaths from 7.8% in 1997 to 3.2% in 2001, with a parallel reduction in the number of brains being retained for formal neuropathology examination within the region. In the ‘Guidelines on Autopsy Practice’ (2002) published by The Royal College of Pathologists it is advised that,
‘All major organs (heart, lungs, brain, liver and kidneys) should be dissected in order to facilitate examination of the blood and drainage in addition to relations with adjacent structures. These organs should be separated and weighed. If permitted and clinically relevant, fixation of the intact brain, followed by a detailed examination by a neuropathologist, produces a higher detection rate of abnormalities.’ [1]
Furthermore, it is suggested that occasions where the contents of the cranial cavity are not examined ‘should be exceptional’. This advice is intended to support a full and thorough approach to autopsy practice at the same time as facilitating training in autopsy techniques.
Phytoremediation of Mercury-Contaminated Mine Tailings by Induced Plant-Mercury Accumulation

In most contaminated soils and mine tailings, mercury (Hg) is not readily available for plant uptake. A strategy for inducing Hg mobilization in soils to increase accumulation potential in plants was investigated to enhance Hg phytoremediation. Accumulation of Hg in the nickel hyperaccumulator Berkheya coddii, the salt-tolerant Atriplex canescens, and the nonaccumulators Brassica juncea and Lupinus sp. was studied by pot trials containing mine tailings treated with either soluble Hg or sulfur-containing ligands. Accumulation of Hg in shoots of B. coddii and A. canescens after addition of soluble Hg was lower than 10 mg/kg dry weight. The addition of ammonium thiosulfate (NH4S2O3) to tailings mobilized Hg in substrates, as indicated by the elevated Hg concentrations in leachates from the pots of both species. Ammonium thiosulfate caused a significant increase in the Hg concentration in shoots of B. juncea. Conversely, Hg translocation to Lupinus sp. shoots was significantly reduced in the presence of this ligand. Mass balance calculations revealed a significant fraction of Hg was lost from the system. This unaccounted-for Hg may indicate Hg volatilization. The results suggest that there is potential for induced plant Hg accumulation for phytoremediation of Hg-contaminated sites. Issues of Hg leaching and volatilization, however, need to be addressed before this technology can be implemented in the field.

We investigated interactions between plant roots, protozoa and nematodes after addition of patches containing inorganic or organic nitrogen in order to determine whether root proliferation could explain the capture of N by the plant from the patch. Decomposition of a 15N/13C, dual-labelled, organic patch in the absence of plant roots was also examined. In the decomposing patch the amounts of 13C and 15N remaining co-varied and both declined with time. Nematode numbers increased. However, protozoan biomass and inorganic N (NO3− and NH4+) availability did not significantly alter as decomposition of the patch progressed. Addition of inorganic N patches, as NH4NO3 solutions, to the first lateral to emerge from the main seminal root axis of Lolium perenne L. seedlings had no effect on root growth compared with controls 16 d after addition. Protozoan biomass increased. Furthermore, log protozoan biomass and NO3− concentrations of the growth medium were significantly (P<0·05) and positively related. Plant response (i.e. biomass production, N capture and root length) to an added organic patch was examined using five different grass species (Festuca arundinacea L., Phleum pratense L., Poa pratensis L., Dactylis glomerata L. and L. perenne). Total plant biomass was significantly (P<0·05) repressed by an organic patch. Plant N content was reduced when an organic patch was present but N concentrations were greater. Roots were generally slow to proliferate within the patch but there was a significant (P<0·05) species×patch interaction for root length within the patch at harvest and in the 2-cm band below it. However, 15N capture by the plants was not related to mean root length duration. All species captured similar amounts of 15N (c. 3–5%) at harvest as a percentage of the initial 15N added in the organic patch. Similarly, the percentage of the total N captured from the patch was not related to the proportion of the root weight within the patch. The fraction of the captured N from the organic patch as a percentage of the plants' total N, however, did differ among species. Substantial amounts (>62%) of the 15N initially added remained in the patch at harvest. Much less (c. 13–21%) 13C remained in the patch. Protozoan biomass and nematode numbers increased significantly (P<0·05) in the organic patch, although the relationship between the two groups was not significant. As in the inorganic N study, the relationship between log protozoan biomass and NO3− concentrations in the soil was significantly positive. We conclude that, when grown in monoculture, plants' N capture from an organic patch is not a simple function of root proliferation. External factors, not plant attributes, are more important in controlling patch exploitation.

1. Quantitative selenium metabolism has been studied in normal young New Zealand women by measuring total Se intake and urinary and faecal Se output, and by using values for absorption, excretion and turnover of 75Se determined after administration of [75Se]selenomethionine or [75Se]selenite.
2. In a period of 14 d when a normal ad lib. diet was being consumed, mean dietary Se for four women was 24.2 μg/d, mean urinary Se was 13.1 μg/d and mean faecal Se was 10.8 μg/d; mean Se balance during this time was +0.3 μg/d.
3. Intestinal absorption of food Se was 0.76−0.83 of intake (mean 0.79).
4. Whole-body Se was calculated in three different ways; (a) using the specific activity of urinary Se and retained whole-body 75Se; (b) using plasma Se and the occupancy of 75Se in whole-body and plasma; (c) using absorbed food Se and the occupancy of absorbed 75Se in whole-body.
5. Whole-body Se calculated from measurements obtained following the administration of [75Se]seleno-methionine was 4.7−10.0 mg (mean 6.9) using method (a), 4.1−7.2 mg (mean 5.2) using method (b) and 4.3−8.9 mg (mean 6.2) using method (c).
6. Whole-body Se calculated from results obtained after giving [75Se]selenite was 2.7−3.4 mg (mean 2.9) using method (a), 2.3−5.0 mg (mean 3.5) using method (b) and 2.1−3.0 mg (mean 2.6) using method (c).
7. The results of this study indicate that the minimum dietary requirement of Se for the maintenance of normal human health is probably not more than 20 μg/d.

1. The daily intake of selenium by three subjects was supplemented with 100 μg Se as selenomethionine (Semet-Se) or sodium selenite (selenite-Se)/d for 10–11 weeks, or with 65 μg Se as in mackerel (Scomber japonicus) (fish-Se)/d for 4 weeks.
2. Urinary and faecal excretion of Se was measured and also Se concentration in whole blood, plasma and erythrocytes. Measurements on blood were made at intervals after supplementation had ceased.
3. Selenite-Se was not as well absorbed (0.46 of the intake) during the first 4 weeks as Semet-Se (0.75 of the intake) and fish Se (0.66 of the intake).
4. Blood Se increased steadily with Semet-Se, from 0.08 to 0.18 μg Se/ml, but more slowly with selenite-Se, reaching a plateau in 7–8 weeks at 0.11 μg Se/ml. Plasma Se increased more rapidly with Semet-Se than with selenite-Se, so that initially with Semet-Se plasma Se was greater than erythrocyte Se.
5. Daily urinary excretion increased with all forms of supplement, with initially a greater proportion of absorbed selenite-Se being excreted than Semet-Se or fish-Se. A close relationship was found between plasma Se and 24 h urinary excretion. The findings suggested that there was a rapid initial excretion of presumably unbound Se then a slower excretion of residual unbound, loosely bound or bound Se.
6. Total retentions of 3.5 mg selenite-Se and 4.5 mg Semet-Se were large when compared with an estimate of body content of 6 mg Se, derived in another paper (Stewart, Griffiths, Thomson & Robinson, 1978). Retentions of Semet-Se and fish-Se appeared to be reflected in blood Se, whereas for selenite-Se, blood Se reflected retention for only a short period after which Se appeared to be retained without altering the blood Se. This suggested that Semet-Se and selenite-Se were metabolized differently.
7. A double blind-dosing trial with 100 μg Semet-Se was carried out for 12 weeks on twenty-four patients with muscular complaints in Tapanui, a low-Se-soil area. Blood Se increased in the experimental group (from 0.067 to 0.143 μg Se/ml); clinical findings were not conclusive and will be presented elsewhere.
8. Blood Se was measured in New Zealand residents before travelling to Europe or to North America. On return their blood Se was increased, and depending upon the period of time spent outside New Zealand some values reached concentrations found in visitors and new settlers to New Zealand.
9. The results from these studies and the earlier studies of single and multiple dosing have been used to look at the various criteria in use for assessing Se status of subjects. It is suggested that plasma Se be used in preference to 24 h urinary excretion, and in addition to whole blood Se and glutathione peroxidase (EC 1.11.1.9) activity.
